1. Our Privacy Commitment
Advanced Medical Supply (“we,” “us,” “our”) is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you this Notice of Privacy Practices (“Notice”). It explains how we use and disclose your PHI and your rights under HIPAA.
2. How We Collect, Use & Disclose PHI
- Collection: We collect PHI when you place orders, request product support, complete online forms, or communicate with us by phone or email.
- Use: We use PHI to process and fulfill your orders, bill Medicare/insurers, coordinate care with your providers, and comply with legal requirements.
- Disclosure: We disclose PHI to Medicare, your insurance carrier, accredited DMEPOS suppliers, or as required by law (e.g., court order, public health reporting).
3. Your Rights Regarding PHI
- Access: You have the right to inspect and obtain a copy of your PHI.
- Amendment: You may request corrections to your PHI if you believe there is an error.
- Accounting of Disclosure: You may ask for a list of disclosures we have made of your PHI.
- Restriction: You can request limits on how we use or disclose your PHI.
- Alternative Communications: You may request we contact you at a different address or by different means.

4. Breach Notification
In the event of an unauthorized use or disclosure of PHI (a “breach”), we will:
- Investigate and contain the breach immediately.
- Notify affected individuals without unreasonable delay, and no later than 60 days after discovery, per 45 CFR §164.400–414.
- Notify the Secretary of Health & Human Services as required.
- Provide steps you can take to protect yourself (e.g., monitor your accounts, place fraud alerts).

6. Changes to this Privacy Policy
We may update this Notice of Privacy Practices at any time. The revised Notice will be effective immediately upon posting, with an updated “Effective Date.”
